There is a Lord Chancellor and Chancellor of the Exchequer, and probably others, but the names are traditional. Chancellor is normally a high rank and in charge of something.
Ministers are normally members of parliament, although sometimes they are Lords, and a Minister heads most Departments (the Foreign Minister - normally called Foreign Secretary - heads the Foreign Office for example). However, not all Departments have Ministers and not all Ministers have Departments. Also, some Departments have many Junior Ministers.
The main Ministers form the Cabinet, under the Prime Minister, but not all Ministers are included.
The Home Secretary heads the Home Office but there is no particular reason to call him Secretary, it's just tradition. The same with Chief Secretary To The Treasury. He's second in command at the Treasury but again his title is just traditional.
