Is it unnatural to say like "Will you fill/put/write today's date, month,and year on this medical sheet?".
Or when you tell someone not to fill in the date,month,and year,is it unnutural to say like "Please leave the space/item of the date blank?"

Maybe you mean the word personal data? It’s said like this. I believe if you were to ask someone to fill this category out they would have to fill out their birthday and everything. I think I got it? Correct me if I’m wrong again sorry if this is frustrating! I’m really trying!

I’m a little confused? Are you asking how to say the day/month/year? How to pronounce date? I’m sorry I’m trying my best! You’re asking how to fill it in? Wouldn’t it just be the correct day the month and then the year? You may have to help me to understand a little better.

I'm sorry to be confusing you.I mean what to call categolized items such as "Date of birth" I'm not asking how to say the day/month/year,but what to call the item to fill in or circle.

Thank you for your answer.However,"personal data" is not what I'm asking about.Let me cite one example.When you submit a prepared document to someone ,then he asks you to fill in today's data,month,and year,but you can't seem to find a space to write them on on it .Then he says" I'm sorry, this document doesn't have a category of the date ,month and year.I'll give you a new one with the category" In this case , what category didn't he enter into the document?

Understood. For English all I can think of is that he forgot to put the category called “Date”. I’m pretty sure there’s not a certain word for the category. We’re just told to put the date which would be day/month/year. If he forgot this category he probably just needed to make the space or lines for you to clearly fill it in for him. In our school sometimes we will be told to write the “Due Date” on our paper. Which is when that the paper has to be finished by to submit. Maybe this is what you mean as well? I hope I was of any help this time?
