Was bedeutet Are you okay? ?

Sometimes I talk with a American friend. And they say 'Are you okay?' whenever I don't reply. I understand this using on some situation like 'when my friends have some trouble and I want to check about it' until now.
But when they say me 'Are you okay?', I have no idea why they ask me. Because I already replied 'I'm good'(answer to how are you) and I just don't need to answer about last text because I think it's end of text. so I think there is no problem..

In short, I wanna know there is another meaning about 'Are you okay?' or else my friend wanna know i'm in AFK.
