Great question! (and very hard to answer, actually)
I’ve definitely heard that phrase in my life. Some of my teachers in the past, for instance, have used it. And sometimes when I talk to strangers, they use it too. Some parents use it with their children, and sometimes couples use it with each other. It’s a nice phrase. Usually the “dear” is an affectionate way of addressing someone. But personally, I almost never use it and none of my friends use it either.
Some other similar phrases are “Thank you, honey,” “Thank you, sweetie,” etc.
I think the general perception is to associate the phrase with the older generation (older parents, grandparents, etc.), although I’ve heard other people use it occasionally too. So I probably wouldn’t make such a clear cut distinction.
I don’t think I can come up with a rule about when to use it. I think the choice of whether or not to use the phrase depends on the person. Some people like to use it, while others don’t. Oh, I should also mention that some people don’t like being called “dear” (especially by strangers) because it can be perceived as inappropriate, and worse yet, insulting and patronizing, as though the person using the “dear” is in a higher position of power than the listener. Another reason is that “dear” can be used between romantic partners, so some people might feel uncomfortable hearing other people calling them “dear.”
Of course, again, it varies from person to person.
Some other examples:
“Can you get that for me, dear?”
“Don’t worry, dear.”
“Yes, dear.”
“What is it, dear?”
Etc.
I would say the phrase is not that common nowadays based on my experiences with the people around me, but I still hear it sometimes. And I’m pretty sure there are contexts (places, situations, people, etc.) where the phrase is still being used a lot. So I think it just depends on the person you talk to.
