That's sometimes a little bit difficult, but I'll give you a rough outline.
"Ursache" (as denote by "ur-") refers to the underlying cause of something. It is thus frequently used when you are talking about cause and effect type of relations. Inversely, it's not appropriate when you are talking about the reason why a person did something:
Warum bist du hier --> Was ist der Grund dafür, dass du hier bist?
(Ursache does not work, and if you would use "Ursache"; the meaning would become "what caused you to go here", as if sth. forced them to visit)
Was ist die Ursache für [historisches Ereignis]?
(you are talking about a complex historic event, and there generally is not only one reason, but a number of them)
Also note that "cause and effect" is translated as "Ursache und Wirkung", so maybe that helps as well.
Grund is generally used when "the cause of an action", as well as "the direct cause of an event" is explained. Especially in this context, many Germans do not really mind whether you use Ursache instead, they do it themselves frequently:
Imagine you just bought ice cream, and it melts too quickly.
Grund could be that it's too hot outside
Ursache (or the "Grund" for the heat) could be global warming
If a tree blocks the street and you are late to work, that's a "Grund", the "Ursache" could be a storm, or the tree being too old and rotten to withstand a mediocre wind.
"Anlass" basically denotes "abc happened, and thus I did xyz", it is also often used as "celebration" or any kind of (good/bad) get-together (wedding, funeral, graduation, ....)
